[BUG] The approval of the plan transitions into a phase as if the plan has been rejected.

What's Wrong?
When I confirm the plan in planning mode and select the first option, which should reset the context and start the plan implementation mode with an empty context, I remain in the same session, and Claude Code tells me that I rejected the plan and asks why I rejected it. The reaffirmation of the plan once again leads to the same consequences as before.
What Should Happen?
It should be a routine action that has happened before: clearing the session, and then the implementation of the plan should begin.

Steps to Reproduce
- enter planning mode.
- discuss and create a plan for implementation.
- once the plan is finalized, Claude Code suggests four options. We choose the first option—session cleanup and plan implementation.
- after that, Claude Code remains in the same session but behaves as if I rejected the plan, asking what the problem is, what’s wrong, and what can be improved, and so on.
- Selecting the implementation option again leads back to the same step, as if the plan was rejected.
