[Bug] Session history repeatedly clearing unexpectedly during active session

Bug Description
i'm not entirely sure why this is happening, but my claude code session history on this session keeps resetting / erasing the history from the screen almost like it's hitting /clear over and over agin, so i can't view any history. this doesn't appear to be hapepning on all my sessions, but it's definitely happening repeatedly on this session
