[BUG] VSCdoe extension doesn't respect edit modes or approved edit modes.

What's Wrong?

No matter what settings I set, i get asked by the vscode plugin for every individual edit to files. THis is awsful.

What Should Happen?

the vscdoe extension should respect my settings for editing.

Load vscode and the claude code extension and then text the different edit modes, they all simply ask over and over again for edit approval.
