[BUG] Claude writing when bypass permissions turned off.

What's Wrong?

Claude continued writing files and running commands even when I turned off bypass permissions. This is clearly a bug and cannot be acceptable behavior.

What Should Happen?

When accept edits is turned off, Claude should not make new files. Simple.

...in any situation where Claude is coding, turn accept edits off. Then notice Claude still writing files.
