[BUG] API Error: Rate limit reached in CoWork only - persistes 24h+, account level issue

What's Wrong?

CoWork returns "API Error: Rate limit reached" immediately on every action.
The error is instant — no processing delay, no partial response.
This has persisted for 24+ hours despite the following:

• Claude.ai chat on the same account: ✅ works normally
• Waited 24 hours: no change
• Restarted Claude, re-opened as Administrator: no change

This is not a local environment issue. It is account-level.

Account Context

• Plan: Pro
• Console usage shown: 4% used
• Console → Limits page: no errors, no flags

Steps to Reproduce

1. Open CoWork
2. Type any message or attempt any action
3. Receive immediate "API Error: Rate limit reached"

What Should Happen?

Expected Behaviour

CoWork processes requests normally within plan limits.

Actual Behaviour

Rate limit error fires instantly on every action, every session.

- #34068 — Same issue persisting 4+ days, account-level ghost rate limit state
