[BUG] Claude Code stops streaming tool activity and only shows “Effecting…” with increasing tokens

What's Wrong?

Claude Code intermittently stops displaying live tool execution and instead shows only:

✽ Effecting… (XXm XXs · ↓ XXk tokens)

No intermediate actions are visible, despite token usage continuously increasing.

What Should Happen?

Showing the files it's reading, modifying or actions doing.
