Problem Statement
Open a feature request describing exactly what you said — approval prompts should have an optional comment/question field so users can ask clarifying questions without breaking the flow
"I'm a new programer operator trying to come up with a code base for my company . When I see a command I don't understand, my only options are Approve or Reject. I suggest a third option: <ask a question before deciding>."
What I can do right now to work around it: I'll always explain commands in a chat message first, then pause and wait for your response before running anything. That way the chat IS your comment field, and the yes/no dialog is just the final rubber stamp after we've already discussed it. That's the workflow I committed to earlier and I'll stick to it.
