[Bug] Excessive token usage with Claude Code Opus 4.6 - max 5x plan, 92% usage in 2.5 hours

Bug Description
extremely excessive limit usage. max 5x plan. 7am central onwards. using opus 4.6. 32% usage in 45 minutes, 60% usage in 1.5 hours, and 92% usage in 2.5 hours. this is beyond throttling. something is wrong.
