What's Wrong?

Version: Claude Code VSCode Extension 2.1.88
OS: macOS (darwin arm64)
Description:
Web sessions appear correctly in the Web tab of the VSCode extension,
but clicking on them throws:
"Error: Claude Code returned an error result: No conversation found with session ID: <id>"
Root cause (from logs):
The extension fetches remote sessions successfully ("Fetched 1 remote sessions")
but spawns Claude with resume: undefined instead of passing the session ID.
This means it starts a new session instead of resuming the existing one.
Workaround:
Using claude.ai/code directly in the browser works fine.
What Should Happen?

Steps to reproduce:
Create a session on claude.ai/code (web)
Open the Claude Code VSCode extension
Go to the "Web" tab — session appears correctly
Click on the session to open it
Expected behavior:
The web session opens inside the extension.
Actual behavior:
Error: "No conversation found with session ID: <id>"
