[Bug] Anthropic API Error: Output blocked by content filtering policy on benign markdown file updates

Bug Description
The claude code keep reject their working workflow (I just let it update .md file in the repo of a recording software (my own software) on the plan/auto mode but it keep reject "API Error: 400 {"type":"error","error":{"type":"invalid_request_error","message":"Output blocked by content filtering
policy"},"request_id":"req_011CZa4PETWwdarDCo6JP57R"} " even there were notthing breaking ToS and this happen my time on many project/repo (I also test with creating a simple react web on next and let it create a markdown file something it reject for no reason)
