[BUG] Session names in the VS Code extension panel keep changing as the conversation progresses.

What's Wrong?

Expected: Name is set once from the first message and stays fixed.
Actual: Name updates multiple times during the same conversation,
making it confusing to identify sessions.

What Should Happen?

Session name should be generated once from the first message and remain fixed for the entire conversation.

Steps to Reproduce

1. Open Claude Code extension in VS Code
2. Start a new conversation session
3. Send a few messages back and forth
4. Watch the session name in the Sessions panel
5. Notice the name changes multiple times as conversation continues
