[Bug] Model switch from Opus to Sonnet via opusplan command not applied

Bug Description
When using opusplan or Option + P to switch model after finishing the Plan mode to Sonnet, the model change does not take effect. Claude code continues to use Opus. Its difficult to notice because the statusline still shows the new model but on the usage and commit messages you can see its Opus
