C:/Program Files/Git/voice mode broken on session start — types spaces repeatedly, toggle off/on no longer fixes it

Description
When starting a new Claude Code session, /voice mode doesn't work — it just types " " (spaces) repeatedly even though it shows as toggled on from the previous session.
Previous workaround (no longer works)
Toggling /voice off then back on used to fix this. As of the latest patch, even that workaround no longer resolves it.
Related issue
This may be related to #39074 (STT transcription never fires). Both involve voice mode failing to produce actual transcription output.
Steps to reproduce
- End a session with
/voicemode enabled - Start a new session — voice mode appears toggled on
- Speak into mic — instead of transcription, spaces are typed repeatedly
- Toggle
/voiceoff, then on again — previously fixed it, now does not
Expected behavior
Voice mode should either work immediately on session start, or cleanly re-initialize when toggled.
Environment
- Windows 11 Home 25H2
- Claude Code latest patch (as of March 25, 2026)
- Hardware: Alienware m18 R2, i9-14900HX
