[BUG] [VS CODE EXT] Claude says he's going to do something and then doesn't

What's Wrong?

Claude says he's going to do something, and then doesn't. Either after accepting a plan or after telling it to do something. I don't know if it's a CC or VS Code extension problem.

What Should Happen?

Claude can't bail on doing things.
