[Feature Request] Standardize approval prompt options to consistent numbering

Bug Description
The claude "approval" options show (1, 2 or 3) OR (1 or 2). When approving it would be easier if "no" was always 3. Otherwise it is easier to accidentally type 2 thinking we are "always approving" a pattern but in this case it is 2 = "no"
