[BUG] When changing Claude Code review settings, active PRs don't change their settings

What's Wrong?

I opened a PR using the new Research preview of Code Review. By default, it had the settings to comment after every push. I changed the settings to just on PR creation. Active PRs during the change still left comments after every push.

What Should Happen?

After the settings change, it should not have commented after future pushes.

Steps to Reproduce

1. Open a PR in a repository
2. Open Claude Review for this repository. By default, it will comment after every push.
3. Change the setting to after PR creation.
4. Push a new change to the active PR from step 1
