Hotkey 2 maps to 'no' on yes/no prompts, conflicts with 'allow all' muscle memory

Permission prompts: 1 = allow once, 2 = allow all, 3 = deny.
Yes/No prompts: 1 = yes, 2 = no.
Users build muscle memory for 2 = permissive option, then accidentally reject yes/no prompts.
Simplest fix: remap yes/no to 1 = yes, 3 = no, 2 = no-op. Rule: 2 is never the negative action.
Better fix: also support letter keys (y/n/a) alongside numbers on all prompt types.
