[BUG] After a delay, plan is auto accepted and feedback is lost

What's Wrong?

Claude for Mac on the Code tab automatically accepts its own plan when I provide feedback (and specifically do not accept the plan) after a delay when the session needs to be re-initialized. I believe this has happened in other cloud sessions (e.g. clau.de/web or iOS) but can't say 100% for sure.

What Should Happen?

  • Claude updates the plan to say it will add the plan to a document to my documents folder, as per my instructions and prompt me to approve _that_ plan.

Steps to Reproduce

I am currently on Desktop in a cloud session (using a roughly default Test environment to setup a postgres database and install some Playwright dependencies via session start hook)

  • I work in planning mode for a while
  • Claude presents a plan
  • I step away for some time
  • I come back to my computer, and type in feedback into the plan (in this case, I told it to write the plan to my repo, rather than implement it)
  • Claude shows "Initialized your session" (in that light gray font)
  • Claude shows "The plan was already approved. Let me implement it."
  • Claude proceeds to implement the plan

I am using Claude for Mac on the code tab. In addition to this issue, I have found it a bit difficult to figure out how to report the issue. It isn't clear to me if this is the right repo to report those bugs or if it is just for the CLI. I have raised this issue with Fin who wasn't really helpful. Our session id is 215473586374483.
