[BUG] Claude Code deletes the plan if it is not accepted

Bug Description

I recently upgraded to Claude Max plan. I've been using Pro for a few months now and it's like multiple software engineers working for me.
One issue I'm facing intermittently is when I get a plan and I want to make some modifications to it before moving ahead and select option 2 "No, keep planning", the plan disappears. Can you check and fix?
