[Bug] Permission request for cached skill reference files in worktree context

Bug Description
After invoking a skill, Reading a skill reference file (~/.claude/plugins/cache/alignment-hive/autopilot/0.1.3/skills/resolve-denied-toolcall/references/adding-permissions.md) triggered a permission request while working in a git worktree under .claude/worktrees/ (entered during the session). Expected this to be auto-allowed
