[BUG] "Shift+Tab" mode change reverts after a few seconds despite defaultMode: bypassPermissions in settings."

What's Wrong?

For some reason, in certain sessions, the “bypassPermissions” mode reverts to the “accept” mode. Once this happens in a session, even if I set the “bypassPermissions” back it will revert to the “accept” mode after a few seconds.

What Should Happen?

if claude code start in bypass mode it should stay in this mode - this bug started with latest versions from few days ago

just create new sessions , it happening random not on all sessions
