[Bug] Claude fails to detect open PRs when worktree branch name differs from upstream tracking branch

Bug Description
When worktree branches are pushed to origin with different names (because the default name is ugly) then Claude doesn't detect open PRs. Claude doesn't associate the upstream tracking branch and instead appears to rely on the branch name explicitly.
