Title: Tool approval dialog not forwarded to remote control client in some cases

Description:
While using /remote-control, some tool approval prompts did not appear on the remote client side. The session was active and working (prior tool approvals, including a git commit and push, were correctly forwarded to the remote). However, a subsequent git log command approval prompt did not arrive at the remote client — the session appeared stuck with no dialog shown remotely.

Steps to reproduce:

  1. Start a session and connect via /remote-control
  2. Approve several tool calls normally through the remote client
  3. Continue the session — at some point a tool approval prompt stops being forwarded to the remote client
  4. The remote screen shows nothing; the local Claude Code interface shows approval pending

Expected behavior: All tool approval prompts should be forwarded to the remote client while the remote control session is active.

Environment: Windows 11, Claude Code CLI
