[Bug] Repeated permission prompts for directories in repo despite "Always Allow" selection

Bug Description
It keeps asking me for permission to access folders at repo root over and over again, despite choosing always allow!

This is happening consistently when Claude pipes CLI tools' output to the dir to read it, for example.
