Steps to Reproduce

1. Use Claude Code normally
2. Error appears intermittently — not tied to a specific action
3. Retrying the same request sometimes succeeds

Expected Behavior
Request completes successfully or returns a meaningful, actionable error code.
Actual Behavior
500 Internal Server Error halts the session with no recovery path other than manual retry.

Additional Context
