/model and /status show stale pricing info for Claude Max subscribers

Description

As of this week, Claude Max includes 1M context windows at no extra charge. However, two places in Claude Code still show outdated information:

1. /model — When selecting a model, it still warns that extended context is "charged extra," which is no longer accurate for Max subscribers.
2. /status — Shows the subscription tier as "Claude Pro" even when the user is on Claude Max.

Expected Behavior

• /model should not show an extra-charge warning for Max subscribers (or at minimum, reflect that 1M context is included with Max).
• /status should correctly identify the subscription tier as "Claude Max" when applicable.
