[Feature Request] Add "Comments/Changes" option to plan approval dialog

Bug Description
When Claude has made a plan, the choices Yes | Yes + some option | No should expand to include something like "I have comments or changes". Frequently I think, well, sure, build it that way and I'll modify when you're done. but it seems better to say, "Add search to all tabs, not just the first two." etc. A clearer User Reaction to Plan would be good. I usually say No and add comments, which is fine for short changes, but not for architecture or inter-dependent changes.
