[Bug] Claude ignores user input upon ESC cancellation, spins wheels instead of reading important context

Bug Description
It used to be that you could ESC out of Claude's pending action, and then explain why at the prompt, thus providing useful context correcting course. Now, when you hit ESC, Claude goes off on all these loops, not knowing/guessing why you have rejected the suggestion, and creating a lot of useless noise, while your explanation like "you'll need uv for that" or "the file you are looking for is here" is sitting there not being read until it's finished spinning its wheels uselessly. It's infuriating!

Suggestion: stop when action is rejected and wait for user's course-correction (like it used to).
