[FEATURE] suggest allowing read access to sub-paths in websites, not just domain names

Problem Statement

I do _not_ trust all content on github, but I _do_ trust some GitHub orgs. I would like claude to prompt me like "Yes, and don't ask again for github.com/anthropics" instead of "Yes, and don't ask again for github.com"

Proposed Solution

when claude repeatedly asks permissions to the same path in a domain, suggest always allowing that path and not the whole domain
