Problem Statement

Auto-loaded files (CLAUDE.md, MEMORY.md) are not tracked for change notifications. Agents working in parallel do not get notifications when these files are updated unless explicitly read, meaning they can be working with stale content.

Proposed Solution

Auto-loaded files (CLAUDE.md, MEMORY.md) should be tracked for change notifications the same way explicitly read files are.
