[BUG] Claude asks approval before actually showing the plan it created

What's Wrong?

When using ExitPlanMode, the plan file content should be visible to the user before they're asked to approve, but the timing seems off — I only see it after dismissing the prompt. That makes it impossible to actually approved of the plan

What Should Happen?

Claude used to show the plan (dimmed) behind the prompt, so I could scroll up and read it before answering the question.
