[BUG] Remote settings not getting saved, re-triggering env var approval every hour

What's Wrong?

We have managed enterprise settings that set OTEL env vars. We're having issues where, for some users, the approval is getting re-triggered every hour despite hitting 1 to trust these settings. If you look inside their ~/.claude/ there is no remote-settings.json being saved. Every hour, when settings are synced, their sessions stop and require approval. This happens when there are no changes made to the managed enterprise settings.

If anyone else is seeing this and has debugging steps, please share. We've tried reinstalling Claude Code with a fresh ~/.claude folder but no luck. We have checked that permissions are writable.

What Should Happen?

The user should only have to approve settings once - unless we change the managed settings.

You see to set the OTEL settings via managed enterprise settings. Other than that, we can't figure why some users have issues but not others.
