[FEATURE] Disable plan in new tab

Problem Statement

Since last update, in VS Code the planning mode opens a new tab. This is a total killer, having to jump between tabs while planning to find previous information is awful.

This should be optional and configurable, and as far as I can see there is no way to disable it, I had to roll back to a previous version. Please add a setting to enable/disable this mode.

Proposed Solution

Add a setting to mantain the plan mode into the same conversation tab.

Alternative Solutions

Remove the new "feature" with the separated plan mode.

1- Working on a large scale refactor where I have discussed many points with Claude, I enter plan mode
2- To review the plan and ensure that matches all what has been discussed I must jump between tabs again and again.
