Problem Statement

Title: VSCode extension: File attachment dialog cannot browse all folders

Body:

When using the paperclip (attachment) button in the Claude Code VSCode extension, the file picker cannot find files outside the workspace. It would be helpful if the file picker allowed browsing all folders on the system (Desktop, Downloads, Pictures, etc.), similar to a standard OS file dialog.
