"Yes, allow all edits during this session (shift+tab)" re-prompts on every edit to same file (v2.1.63)

Preflight Checklist

  • [x] I have searched existing issues and this hasn't been reported yet
  • [x] This is a single bug report (please file separate reports for different bugs)
  • [x] I am using the latest version of Claude Code

What's Wrong?

While using CC every new call which requires edits on files re-prompts for permission, even if I select "Yes, allow all edits during this session (shift+tab)" for that particular file.

I am not using sub-agents.

  • Environment info: win32, windows-terminal, v2.1.63

What Should Happen?

Ask for permission once for each file and then store this permission when selected the Allow all edits for this session for that file.

Steps to Reproduce

Steps to reproduce:

Start Claude Code normally (no flags) in Git Bash (MINGW64) on Windows
Ask CC to edit a file (e.g. index.ts)
When prompted, select "Yes, allow all edits during this session (shift+tab)"
Ask CC to make another edit to the same file
Permission prompt appears again despite selecting "allow all edits during this session" in step 3
