[Bug] Remote control session state desync between VS Code and Android app with Anthropic API 500 error

Bug Description
Desync between Claude Code in VS Studio with Remote control activated vs. Claude Android App. My Android App is stuck on messages from some days ago... every time it reconnects it asks me to Accept / Deny tasks that were ALREADY executed in VS Code, and whatever i say (accept or deny) it will repropose these questions / tasks to me next time I open app and connect to code session, and won't show me the real progress of the session (from what's happening on VS Code). I think it was linked to this weid error message I received during one session in VS Code: API Error: 500 {"type":"error","error":{"type":"api_error","message":"Internal server error"},"request_id":"req_011CYVpueCy...[redacted for privacy... this is a public github repo]"}
