Same here. It happened in the middle of executing a plan while using Claude CLI.

It also happens in the CLI version of Claude Code:

ensure you updated by restarting the claude desktop app . just clicked relaunch and now it works

I don't know if any of you had opus 4.6 took really bad decision(not listening mostly) in the last hours like it was for me
