[Bug] Model effort level not displayed correctly on initial selection

Bug: When selecting a model in /model that already has high effort/thinking enabled, the confirmation message omits the effort level:

"Set model to Default (Sonnet 4.6 · Best for everyday tasks)"

It should read:

"Set model to Default (Sonnet 4.6 · Best for everyday tasks) with high effort"

Steps to reproduce:

  1. Enable high effort/thinking on Sonnet 4.6
  2. Open /model and re-select Sonnet 4.6
  3. Notice the confirmation message is missing "with high effort"

Workaround: Manually moving the effort slider (even back to the same position) causes the correct message to appear.

This is a display-only bug — the effort level is set correctly, it's just not reflected in the initial confirmation message.
