[Bug] Claude ignores instructions to exclude ANSI escape sequences from commit messages and PR descriptions

Bug Description
Despite all the places in my CLAUDE file and other skill files etc where I have explicitly told it NEVER to use ANSI/Terminal escape chars in commit messages or PR descriptions, WITHOUT FAIL it almost always includes them anyway. Not sure if this is because of my terminal setup or what, but I am sick of wasting tokens telling Claude to go back and fix this.

This is a longstanding issue that has been present since the release of Claude Code.
