[BUG] Claude's task tool creates files that Claude is not aware of

Bug Description

It happened twice today that i asked claude to create an implementation plan, and while doing that he produced a markdown file in a completely unexpected location (project root) that claude seems to then use to write the plan. In both cases I asked claude why it produced two files. In both cases Claude seemed surprised that the other file existed. Even claimed in the second instance, that he did not create the file. After telling Claude I just saw him create it, he said that it seems like the task tool must have created it.
