[Feature Request] Add granular control to disable "always allow" for complex bash commands

Question

**Bug Description**
"There's no setting to tell Claude Code "never offer always-allow for complex commands." It treats all bash commands the same.
  It's a UX gap — it probably shouldn't offer "always allow" for heredocs and multi-line commands, but currently it does."
