"API Request Failure: Empty Custom Command File Causes Persistent 400 Error"

Bug Description
An empty custom command file will brick claude code. If you forget to save the file contents before launching, claude will send the empty prompt to the API which returns a 400. Since this is now in the chat history, all subsequent requests will also return a 400 effectively bricking Claude Code until restarting.
