Confusing error with saved screenshot

**Bug Description**
I tried to place a screenshot in a directory called screenshots/ and asked Claude to review it. I got this error and Claude Code completely stopped responding to me, everything I ask I get this error back: API Error: 400 {"type":"error","error":{"type":"invalid_request_error","message":"messages.92.content.0.tool_result.content.0.
