[BUG] Organization skill not loading in Claude Code session despite being enabled

What's Wrong?

An organization skill ("wv-theme") that was added and enabled by an admin over a week ago is not being recognized or loaded in Claude Code sessions.

What Should Happen?

The organization skill should appear in the available skills list and be invocable via the Skill tool.

Error Messages/Logs

Steps to Reproduce

1. Admin adds and enables an organization skill under "Organization skills" tab
2. Skill appears in the Skills settings as enabled (toggle on)
3. Start a new Claude Code session
4. Attempt to invoke the skill - returns "Unknown skill: wv-theme"
5. The skill does not appear in the system reminder listing available skills

Troubleshooting attempted:

1. Restarted Claude Code (full quit and reopen)
2. Toggled the skill off and back on (re-synced)
3. Started multiple new sessions
