[FEATURE] /rename should work during activity

Problem Statement

The /rename command is great, but it only works when Claude Code is not actively working. Very often I want to invoke it during the middle of an active session, because I want to "tag" it and move on to another session.

Proposed Solution

/rename should work during active sessions (e.g while Claude is planning, coding etc)
