[BUG] Renamed session not findable by new name in session list

What's Wrong?

After renaming a session using /rename, the session cannot be found under its new name when trying to resume it from the session list.

What Should Happen?

The session should be listed and findable under its renamed name.

Steps to Reproduce

1. Start a new Claude Code session
2. Rename it using /rename ts_datenmodell
3. Confirmation appears: Session renamed to: test_session
4. Exit the session
5. Try to resume the session - it does not appear under the name test_session in the session list
