[BUG] With a custom plan name, claude automatically executes the plan after updating it

What's Wrong?
When the plan has a custom name, and is in a plans folder specified in the project config, then to make changes to it Claude asks to exit plan mode. It says "Can I exit plan mode to apply these changes to the plan" but as soon as it exits plan mode, it starts executing the plan.
What Should Happen?
It should edit the plan and stay in planning mode. It shouldn't need to exit planning mode in order to edit a folder in the settings specified plans folder, or a plan with a name which relates to the plan content instead of a random name.

Steps to Reproduce
- Set a plans folder in your claude settings
- Go into planning mode
- Discuss a plan
- Tell Claude to name the file appropriately instead of randomly (so it gets a specific name)
- Now tell Claude to change something in the plan
