[BUG] Extension host terminated unexpectedly 3 times within the last 5 minutes (VS Code)

What's Wrong?

For the past week your VS Code extension keeps crashing VS Code, it seems to have happened after you had that Opus 4.5 issue last week. As soon as I click on the claude icon, it opens the UI and then I get the error and cannot use the extension. I am on windows 11 pro, latest VS Code and latest claude Code.

I have tried uninstalling and reinstalling etc... But nothing works. It's now unusable in VS Code which is how I like to use this and very annoying as I pay for the x20 max plan!

What Should Happen?

Not crash VS Code.
