Inconsistent Plan Generation: Varying Output Formats and Actionability

Question

**Bug Description**
The output from plan mode is inconsistent. Sometimes, it generates a clear, step-by-step plan that's ready for execution, which is great. However, other times it just describes its findings without outlining an actual plan. On other occasions, it provides a detailed analysis of my request rather than a direct, actionable plan for itself to follow. This makes the experience a bit unpredictable.
