Bug Report: API Error 400 - Tool Use Concurrency Issue
Description
Encountered API Error 400 due to tool use concurrency issues. The error message suggests running /rewind to recover the conversation.

Platform: Linux
Terminal: VS Code
Version: 2.1.19

Actual Behavior
API returns 400 error with message: "due to tool use concurrency issues"

Additional Observations

Opus 4.5 appeared to underperform compared to expectations in terms of overall reasoning quality. This may or may not be related to the concurrency issue.
The model occasionally hangs without producing any output or consuming tokens:

After waiting over 10 minutes no tokens were used and no response was generated.
